The Clinical Nurse Specialist will be required:
- To provide advice, education and support to liver patients, their families and care support networks.
- To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
- To undertake hepatology nurse-led clinics and to be responsible for high standards of nursing care through planning, coordinating, delivering, and evaluating nurse-led clinics.
- To work autonomously to deliver research-based, specialist hepatology clinical care that enhances patient outcomes.
- To contribute to training and education within the hepatology specialty in hospital and community settings.
- To promote continuous improvement of patient care by reviewing evidence and engaging in research activities.
- To act as a visible hepatology role model and expert practitioner.
Main duties of the job
- Be a point of contact for patients/carers for specific care pathways, providing clinical care, advice, and psychosocial support.
- Assess health and psychosocial needs, providing care in partnership with patients and colleagues, and referring as needed.
- Deliver expert nursing care within agreed settings, supporting treatment assessment, monitoring, and management of liver conditions.
- Escalate issues appropriately within professional judgment.
- Communicate effectively to empower patients in health decisions.
- Act as an advocate for patients and their families.
- Provide education to promote health and self-care.
- Develop and maintain patient information and care plans, ensuring documentation meets NMC standards.
