Job description

The Clinical Nurse Specialist will be required:

1. To provide advice, education and support to liver patients, their families and care support networks.
2. To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
3. To undertake hepatology nurse-led clinics and to be responsible for high standards of nursing care through planning, coordinating, delivering, and evaluating nurse-led clinics.
4. To work autonomously to deliver research-based, specialist hepatology clinical care that enhances patient outcomes.
5. To contribute to training and education within the hepatology specialty in hospital and community settings.
6. To promote continuous improvement of patient care by reviewing evidence and engaging in research activities.
7. To act as a visible hepatology role model and expert practitioner.

Main duties of the job

1. Be a point of contact for patients/carers for specific care pathways, providing clinical care, advice, and psychosocial support.
2. Assess health and psychosocial needs, providing care in partnership with patients and colleagues, and referring as needed.
3. Deliver expert nursing care within agreed settings, supporting treatment assessment, monitoring, and management of liver conditions.
4. Escalate issues appropriately within professional judgment.
5. Communicate effectively to empower patients in health decisions.
6. Act as an advocate for patients and their families.
7. Provide education to promote health and self-care.
8. Develop and maintain patient information and care plans, ensuring documentation meets NMC standards.
