Heart Failure Team Health Support Worker

The closing date is 08 October 2025

  • Support the ACERS Heart Failure and Rapid Diuresis Response/Virtual Ward service with the delivery of care and administrative processes.
  • The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings.
  • Implementing and monitoring their care needs under the supervision of registered nurses.
  • The post holder will take responsibility for their own actions when delivering delegated clinical care, maintaining competency and skill in line with the role requirement.
Main duties of the job
  • The post holder will provide excellent communication skills with patients, relatives and other MDT members.
  • Work closely with the team clinicians to efficiently process new referrals, inquiries, and collate information.
  • Support the ACERS Heart Failure service with maintaining effective administrative processes.
  • The post holder will be required to work across multiple sites.
  • Undertakes delegated clinical / technical tasks following competency assessment (i.e. phlebotomy, ECG recording).
  • To undertake delegated clinical observations including vital signs, blood sugar, skin integrity assessment.
  • To identify changes in patients' condition and, within their capabilities, recognise where significant changes in the care plan are required and report back to senior clinicians in a prompt and timely manner.
