- Lead and line manage the established community heart failure team.
- Provide advanced clinical triage of new referrals and assess/treat heart failure patients.
- Determine clinical diagnosis and treatments as an autonomous practitioner.
- Support virtual ward and hospital-at-home care from a heart failure perspective.
