Heart Assessment Team Sister/Charge Nurse (Band 6)

Join to apply for the Heart Assessment Team Sister/Charge Nurse (Band 6) role at The Shrewsbury and Telford Hospital NHS Trust.

An exciting opportunity has arisen within the Shrewsbury and Telford Hospital for a Heart Assessment Team Sister/Charge Nurse managing heart failure and cardiac rehabilitation patients. The vacancy is based at The Royal Shrewsbury Hospital and is a permanent position for 30 hours per week. There may be a need to work across sites.

Responsibilities include:

  1. Providing ongoing support and lifestyle advice to cardiac patients in inpatient, outpatient, and community settings through one-to-one counseling and group sessions.
  2. Undertaking patient assessments before cardiac rehabilitation exercise programs.
  3. Managing nurse-led clinics focusing on risk assessment, medication support, urgent symptom review, and palliative care.
  4. Collaborating within a multidisciplinary team to promote partnership working between primary and secondary care providers.
  5. Implementing evidence-based clinical practices to support patients with heart failure and cardiac rehabilitation needs.
