Job overview

Skylark Rehabilitation Unit is a 30 bedded facility that is located Yate Bristol. The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess pathway 2 (please see below).
Following a stay in the acute setting, the ethos for all people being discharged is ‘home is best’ and a large number of people are supported to return home utilising the robust and established ~Discharge to Assess Pathway 1. For those people that are identified as having on-going rehabilitation needs, they are considered to ‘stepdown’ into a Pathway 2 Rehabilitation, Reablement and Recovery bed at the Skylark Rehabilitation Unit.
On the Unit, they will receive therapy intervention and support around complex discharge planning, to enable a timely and safe discharge to their usual residence or to a more suitable location if appropriate.
We actively promote a multi-disciplinary model on the Unit, where nursing staff work closely with therapist, social workers, GP’s with Special Interest in Older People’s Care as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision

Main duties of the job

The Health Care Assistant role compliments and forms a key part of the multi-disciplinary healthcare services within the organisation, implementing and providing high standards of individualised care for the service user. You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, occupational therapists, physiotherapists and other allied healthcare professionals.

• The Health Care Assistant will provide holistic, person-centred care and support for service users of all ages in a variety of settings. As a member of the MDT team you will support the registered nurses in the delivery of care to service users and contribute towards the safe and seamless delivery of holistic care, working safely within your scope of practice and competence at all times.

• Your primary purpose will be to carry out routine care and health tasks which will be set out in agreed care plans as directed by the registered nurse. These tasks will include the provision of personal and nursing care and other practical support that will help service users recover and / or maintain their independence.

• You will deliver clinical care, particularly observation, blood glucose monitoring and simple dressings to service users on the unit under the delegation, support and guidance of a Registered Nurse
