Role responsibilities
  • Monitor patients\' conditions by taking vital signs and escalate changes to the district nurse or senior nurse on duty
  • Take blood for testing (phlebotomy) and administer insulin following appropriate training and consolidation
  • Carry out catheter care and skin integrity checks
  • Carry out simple wound dressings and other nursing tasks as assigned by the senior nurse
  • Promote patient independence through implementing rehabilitation programmes as directed by the multi-professional team
  • Undertake health promotion activities with patients and their families as directed by the District Nurse or multi-disciplinary team
  • Maintain up-to-date stock levels of materials and equipment used by the team
  • Maintain accurate documentation immediately after nursing intervention in line with Trust policies, guidelines and procedural documents
  • Use mobile working devices to record and document relevant information
  • Undertake routine audits as required
Hours and requirements
  • The two shift patterns for the role are 08:00–16:00 and 12:00–20:00, including weekends and bank holidays
  • The post holder must have a full current UK driving licence and daily access to a car, as this is essential for the role
