The Healthcare Navigator role will provide care and support to those in the early days in custody with the aim to add significant value to their experience of the prisoner and prison staff; reduce the risk of self-harm and suicide alongside wider impacts on families and friends.

This role will provide reassurance, guidance and signposting to health and social care services that will contribute to minimising distress and risk for those patients in their early days in custody.

Main duties of the job

• Offer a visible presence during their early days in custody and on the Induction Wing - therefore being accessible and available to all new receptions.
• Help to improve identification of risk factors during early days in custody.
• Be a bridge between HMPPS and the Healthcare providers to enable improvements in joint care.
• Address vulnerabilities associated with early days in custody inclusive of reducing self-harm.

Job responsibilities

• Create greater awareness of healthcare and social care options and the range of services available.
• Support access to those services, especially for people from diverse backgrounds or with specific needs.
• Improve partnership working between services, particularly social care and Safer Custody teams.
• Improve engagement rates into healthcare and reduce did not attend appointments.
• Assist with improving patient choice regarding healthcare interventions.
• Further support social prescribing and promote and manage Health Champions.
• In conjunction with EPIC, establish and run specific patient forums particularly relating to post ACCT.
• Input into complex case meetings.
• Be responsible for logging and making referrals.
• Offer appropriate support and guidance to patients and their families/carers.
• Plan/organise work using own initiative while being able to work as a valuable member of a team.
• Have excellent IT skills, including Microsoft Office, Outlook, and Excel.
• Undertake general office duties to support the role.
• Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise, and support where requested.

Person Specification

Education and Qualifications

• 5 GCSEs at grade C or above in Maths, English, and Science.
• A-level/NVQ level 3 or equivalent.
• Foundation degree/level 1 diploma modules or willingness to undertake.
• Health Promotion Qualifications/Certificates or willingness to undertake.

Other

• The post holder must have the ability to understand and implement the equal opportunities policy at a level appropriate to the job.

• Ability to promote and work in line with the working model of a 7 day a week service.

Skills & Knowledge

• Good organisation and prioritisation skills.
• Good communication skills (written and oral).
• Basic word processing and spreadsheet skills.
• Experience with using SystmOne (Clinical System).
• Ability to motivate self and others.
• Team worker able to relate to all grades of staff.
• Able to work under pressure.
• Appropriate levels of confidentiality are maintained.

Previous Experience

• Experience of working in a team.
• Experience of Health and Safety practices.
• Experience of working to deadlines.
• Previous experience within a secure/prison environment.
