Job description

The Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The HCA will work in patients’ own homes as well as visiting patients in residential homes. The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The HCA will promote optimum independence and individualised client-centred care to a delegated caseload. The post holder will also support the Community Nursing Team to deliver against key performance indicators and maintain quality of the service. The service is provided seven days per week from 4pm to 8pm across four localities in Herts Valleys.

Responsibilities
  • Provide a mixture of general and specific care under the guidance and supervision of a registered nurse or allied health professional.
  • Work in patients’ own homes and visit patients in residential homes.
  • Undertake competency assessments to become proficient and competent in the area of clinical practice.
  • Promote optimum independence and client-centred care for a delegated caseload.
  • Support the Community Nursing Team to deliver against key performance indicators and maintain service quality.
  • Operate as part of Twilight Community Nursing service with flexibility to cover 4pm–8pm shifts, across four localities in the Herts Valleys area.
Requirements / Qualifications
  • Interest in continuing to develop skills in community nursing.
  • Ability to work flexibly according to service needs.
  • Excellent organisational, time management, and communication skills.
  • Willingness to undertake competency assessments and training as part of professional development.
  • Based at one of four sites within the Herts Valleys area: Watford, Potters Bar (Hertsmere), St Albans and Harpenden, and Hemel Hempstead.
