Job summary

A leading healthcare provider is seeking a Healthcare Assistant for the Enhanced Care Team, working 15-18 hours weekly. Healthcare Assistant (The Enhanced Care Team)

We are recruiting for aHCA, to work 15-18hours per week on a permanent contract. The role will include independently undertaking homevisits for the PCN Enhanced Care Team.

This role will require an EnhancedDBS Clearance. Should you have this certificate as part of the DBSsubscription service, we would be happy to accept this providing the originalis shown. Otherwise, you will be expected to complete a new application.

Main duties of the job

The successful applicant will independently complete Long-term condition reviews and Frailty Assessments. They will also help the team with demand for phlebotomy & HCA services.

Job responsibilities

Workingin the community with housebound patients under the direction of a qualifiednurse.

Tocollect venous/capillary blood samples without harm to patient or self byfollowing the phlebotomy standard operating procedures.

Toobtain such blood samples to a standard fit for laboratory testing ensuringcollection, maintaining appropriate files & records as required.

Toensure that all phlebotomy equipment and accommodation is properly maintainedand suitable for use.

Tocompetently complete Frailty assessment visits.

Tocompetently complete Long Term Condition reviews.

Undertake ECGs

Dopplers

Wheelchair assessments

Basic observations

Measurement forstockings

Collection ofprescriptions

Person Specification
Experience
  • Previous experience of working in a Primary Care
  • Previous HCA or Phlebotomy experience in a healthcare setting
Qualifications
  • GCSEs or similar, to include English Language and Maths
  • Qualified in Phlebotomy
  • NVQ level 2 or Level 3 in Health and Social Care
Knowledge and Skills
  • Be able to prioritise their day and be familiar with all routine blood tests and understand their clinical importance
  • Strong communication in English, both written and spoken.
  • To deal with distressed patients in an empathic, caring and compassionate way.
  • To write and maintain accurate clinical records.
  • Effective time management

  • To demonstrate and be up to date in competency and understanding of phlebotomy technique and sampling conditions
  • To be able to work independently within a team structure and possess excellent communication skills
  • Demonstrate a flexible approach to work and to follow instructions and guidance given to them
