The Healthcare Support Worker will provide high-quality and compassionate care to individuals. They will have the understanding and ability to deliver delegated care under supervision and support the multidisciplinary team in delivering high-quality care to help individuals achieve their person-centred goals in their own homes.
The post holder will carry out routine duties in accordance with the individual's care plan, promote independence, and monitor the individual's progress, comfort, and wellbeing.
When working as a lone worker, the post holder will act on their own initiative to ensure safety and report concerns as required.
Kingswinford & Wordsley Primary Care Network is a network of 5 practices serving over 50,000 patients. This role supports the delivery of the PCN's agenda and is part of the leadership team working alongside Practice Managers, the Clinical Director, and PCN Partners.
Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions.
Plays a key role in vaccination and immunisation programmes within the PCN.
Works within a multidisciplinary team to support patients with long-term conditions and health inequalities.
Provides clinical support to registered nurses and other healthcare professionals within the PCN.
Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives.
