Requirements:

• Availability from 7am-2pm or 4pm-10pm

Main Duties and Responsibilities include:

• Coordinate with Social Workers, District Nurses, and other professionals within the multidisciplinary team when required
• Communicate changes in requirements effectively
• Adhere to company policies and procedures
• Attend training and pursue personal development
• Assist customers with personal needs such as dressing, washing, meal prep, and community access as per the Care Plan
• Assist with light housework and meal preparation in customers' homes as needed
• Encourage and support customers to undertake personal care and tasks they are capable of, recognizing that abilities may vary daily
