Cranstoun Group is seeking a dedicated professional to join their community team in Sandwell, focusing on delivering a new Diamorphine Assisted Treatment program. The role involves providing support to service users with substance misuse issues, conducting health assessments, and working closely with medical staff. Salary: from £25,630 – £30,490 per annum dependant on experience
Hours: 35 per week, Monday – Friday (potential for weekends/bank holidays available as overtime)

Cranstoun Sandwell in partnership with the West Midlands Office of Police and Crime Commissioner and Sandwell Metropolitan Council launched a brand new Diamorphine Assisted Treatment (DAT) pilot programme in 2022. This was a 2 year pilot which has been extended and is proving to be an exciting opportunity for us to work in a new way with opiate dependant service users who have not regularly engaged with traditional treatment services. The DAT project delivers all of our usual offer in addition to daily supervised Diamorphine treatment and associated specialist interventions.
Having experience and knowledge of supporting people with substance misuse, including the complex health needs this client base often present with, you will bring knowledge and commitment to deliver the range of community models of drug and alcohol treatment including experience with delivering psychosocial interventions for this client base. You will have a non-judgemental attitude and a desire to support the people who use our services to feel healthier and live safer by delivering a high-quality service.
The role will include an element of healthcare assistant supporting medics alongside a keyworker aspect delivering psychosocial interventions. You will be involved in the assessment of service users, goal planning, blood borne virus screening, other harm reduction interventions, health assessments, linking with partner agencies and supporting service users to access help for sometimes complex health needs and supporting the administration of controlled drugs alongside a specialist nurse. You will complement the clinical team members in managing the health needs of service users on the DAT programme as well as supporting people to achieve their goals through care planning and one to one sessions. The role will include work on base and on occasions some outreach across Sandwell area.
The successful candidate will be committed to a strong harm reduction approach and encourage people who use the service to utilise their recovery capital within their community. There will be a need to develop effective working relationships with your colleagues, service users and external partner agencies across the borough. The desire to ensure the effective delivery of the DAT programme in Sandwell is paramount and you will need demonstrable skills in safe and thorough case management.

There will be a requirement for flexibility and for weekend work on a rota basis as the DAT programme will be delivered 7 days a week, 365 days a year. This role will be fixed term until 31 st December 2025.
