Job description

You must have been in the UK for 12 months prior to applying for this post and be able to provide a 6-year address history to complete the prison clearance.

This role is subject to an additional recruitment and retention premium of £1542 per annum.

An opportunity has arisen for an experienced senior support worker to join a vibrant mental health inreach team within Health and Justice based at HMP Manchester. The post holder will work within the MHIT and will be an integral part of the wider clinical MDT.

This is an exciting opportunity to be part of an established service within the Health & Justice directorate of Greater Manchester Mental Health NHS Foundation Trust. You will be supported through the security checking procedure (Counter Terrorism Clearance) to work in a High Secure Environment. Throughout your employment, you must remain compliant with renewal checks every 5 years.

The post holder will assess physiological observations, including blood monitoring and ECGs, and promote healthy lifestyles for our service users. They will assist in enabling service users to access support for recovery from mental and physical health issues in a custodial setting. The role includes working with care co-coordinators across boundaries of care to plan, implement, and evaluate care within care pathways as part of the stepped care model.
