A community health organization in the Sheppey area is seeking a compassionate Healthcare Assistant to support registered nurses in delivering high-quality care in patients' homes. We are looking for a compassionate and motivatedHealthcare Assistantto join our Community Nursing team. In this role, you will support registered nurses and senior staff in delivering high-quality care to patients within their own homes and community settings. Youll help provide both clinical and personal care, ensuring patients feel supported, respected, and safe at every stage of their healthcare journey.
This is a full-time position (37.5 hours per week) based across the Sheppey area.
In this role, you will:
Provide nursing and personal care as delegated by the Senior Sister/Team Sister.
Support patients in their homes, assisting with clinical tasks such as dressings, glucose monitoring, phlebotomy, and continence reassessments (following training and competency sign-off).
Accurately record and document care provided, maintaining both electronic and paper health records.
Monitor patient conditions and promptly report any changes to the nurse in charge.
Communicate effectively with patients, carers, and the wider multi-disciplinary team.
Assist with clerical and administrative duties as required.
Actively promote health, safety, and wellbeing for patients and colleagues.
Participate in audits, team meetings, training, and clinical supervision.
Support the orientation of new staff and students.
Help ensure resources, stock, and equipment are well-maintained and used effectively.

We are seeking someone who is:
Compassionate, approachable, and committed to delivering excellent patient care.
Able to work both independently and as part of a team, showing flexibility and resilience.
Skilled in communication, able to listen to and support patients and carers with sensitivity.
Organised, able to prioritise workload and manage tasks without direct supervision.
IT-confident, with an understanding of maintaining accurate records.
NVQ Level 2 in Health & Social Care (or equivalent).
Previous experience in health or social care.
A current driving licence and access to a car.
NVQ Level 3 in Health & Social Care.
Previous experience in community nursing.
Competencies in venepuncture, blood glucose monitoring, catheter and stoma care, and wound care.
