Job summary

Nottingham Forest Foundation seeks a Health & Wellbeing Coordinator to drive community health projects across Nottinghamshire. You will coordinate, monitor, and evaluate programmes, working with a wide range of partners to improve physical health, mental wellbeing, and social inclusion.

In this role you will develop partnerships with NHS and local services, oversee core programmes, and support funding bids to sustain impact. A strong planning, data, and safeguarding mindset is essential.

Qualifications

• Qualification in Health, Wellbeing, Community Development, Coaching, Sport, Public Health, Mental Health, or a related field.
• Lived experience of programmes that support health/wellbeing/mental health including providing advice, guidance, and goal setting.
• Activity delivery (e.g. UEFA C/Multisport) or working towards.

Responsibilities

• Lead on-the-ground delivery of health and wellbeing programmes across Nottingham City and County.
• Engage with partners (NHS, Public Health) to strengthen referral pathways into Foundation-led programmes.
• Coordinate and oversee Healthy Goals sessions and Resilient Reds hubs in priority areas.
• Support budgeting, reporting, data collection, and monitoring aligned with PLF & PFA requirements.
• Develop partnerships, funder relationships, and opportunities for ongoing funding.
