Main duties of the job

As a Health& Wellbeing Coach you will work as a key part of the Primary Care NetworkPCN Multi-disciplinary team, you will consider the whole person in addressingexisting issues and encouraging the proactive prevention of new illnesses,promoting lifestyle changes and adaptations.

HealthCoaching revolves around using your motivational interviewing and coachingskill to support people with lower levels of patient activation to develop theknowledge, skills, and confidence to manage their own health and wellbeingwhilst increasing their ability to access and utilise community support offers.

This is a newrole as part of an existing team at SGPA where you will support a city-wideWaiting Well programme offering targeted support to targeted groups of patientswaiting for surgery in Sunderland. This will involve delivering group sessionsand 1-1 support to patients covering a range of health and wellbeing subjectsto ensure patients are fit and fully prepared for surgery.

The postholder will be responsible for the collection of relevant health data andmeasures to feed into the wider project to evidence patient impact.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

  • HealthCoaches will manage and prioritise a caseload, in accordance with the healthand wellbeing needs of their population through taking an approach that isnon-judgemental, based on strong communication and negotiation skills, whileconsidering the whole person when addressing existing issues. Where requiredand as appropriate, the Health Coach will refer people back to other healthprofessionals within the PCN;
  • Providepersonalised and group support to individuals, their families, and carers tosupport them to be active participants in their own healthcare; empowering themto manage their own health and wellbeing and live independently through:
  • Coachingand motivating patients through multiple sessions to identify their needs, setgoals, and supporting patients to achieve their personalised health and careplan objectives;
  • Providinginterventions such as self-management education and peer support;
  • Supportingpatients to establish and attain goals that are important to the patient;
  • Supportingpersonal choice and positive risk taking while ensuring that patientsunderstand the accountability of their own actions and decisions, thusencouraging the proactive prevention of further illnesses;
  • Workingas part of the city wide social prescribing service to connect patients tocommunity-based activities which support them to take increased control oftheir health and wellbeing;
  • Increasingpatient motivation to self-manage and adopt healthy behaviours;
  • Workwith patients with lower activation scores to understand their level ofknowledge, skills and confidence (their Activation level), when engaging withtheir health and well-being and subsequently supporting them in shareddecision-making conversations;
  • Utilisehealth coaching skills to support people with lower levels of activation todevelop the knowledge, skills, and confidence to manage their health andwellbeing, whilst increasing their ability to access and utilise communitysupport offers; and
  • Exploreand support patient access to a personal health budget, where appropriate fortheir care & support
  • Support& deliver group Consultations to enable patients to better understandtheir LTH conditions, how to manage itand encourage the patient agree to self-management goals, and creating aenvironment for individuals to access peer support

Experience
  • Experience of supporting people, their families and carers in a paid or unpaid capacity.
  • Experience of setting up and delivering Group Consultations within a Health or Community setting.
  • Experience in delivering behaviour change interventions i.e. Smoking Cessation, NHS Health checks, Weight management.
  • Demonstrable knowledge in lifestyle or health interventions for example exercise or nutrition.
  • Experience of supporting people with their mental health or wellbeing needs.
  • Experience of working in a community setting.
  • Experience of handling confidential information.
  • Experience of collecting and recording information and data.
  • Ability to identify risk to self and others, Identifying and reporting safeguarding incidents.
  • Working Knowledge of Long-Term Conditions for example diabetes or cardiovascular disease.
  • Experience of working in or with voluntary organisations or groups in a paid or unpaid capacity.
  • Experience of working collaboratively with different organisations, building trust, confidence and partnerships.
  • Extensive knowledge of local services within a Sunderland Locality through either living or working within one of the wider Sunderland settings.
  • Experience of working with GPs and/or other Health or Social Care providers or knowledge of how systems work.
Qualifications
  • Demonstrable commitment to personal and professional development.
  • Proficient in the use of Microsoft Office applications.
  • Qualification or training in UKHCA Health coaching, or equivalent experience level 3 diploma or above or working towards this.
