Full time (36.5 hours per week)

JOB OUTLINE:

To deliver support to individuals who are currently smokers, undertake behavioral change and practical interventions so that they can set a date for smoking cessation. The service delivery will operate within designated community venues across East Birmingham including Ward End, Firs and Bromford, Alum Rock, Tyseley and Hay Mills, Sheldon, Small Heath and Garretts Green

MAIN RESPONSIBILITIES:

  • To organise own workload and work unsupervised in a busy team.
  • To maintain a high standard of work and prioritise own workload according to the needs of the service
  • To take referrals, including self-referrals into the service and work with individuals to identify the most appropriate tobacco dependency treatment pathway.
  • To respond effectively to referrals within stated timescales – self-referrals from the individuals, and those from colleagues /other workers.
  • To support individuals to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioral changes to improve their health.
  • To motivate and empower clients to make changes to address their addiction
  • To arrange up to 5 contact meetings with individuals to support them in their journey to their quit date. Contact meetings can be face to face/by telephone/via Teams, to meet the needs of the individual
  • To forge strong working relationships with local pharmacies and develop a referral process
  • To refer individuals to a local pharmacy or GP to gain treatment and make additional contact with them six weeks later for updates
  • To complete required paperwork accurately and in a timely manner
  • To update the CRM with appropriate information.
  • To develop good relationships with delivery partners, agencies and services for the benefit of the project and its participants
  • To supply monitoring information to the line manager on a monthly basis.
