Are you an experienced, newly qualified, or returning Health Visitor eager to make a real difference with the children, young people, and families in Tameside?

Our Healthy Child Programme 0-19 (25 SEND) Service for Health Visiting and School Nursing has a vision to improve health outcomes for our population and influence the wider determinants of health, through collaboration with the people of Tameside and Glossop and our health and care partners. We are currently progressing through an exciting service transformation in conjunction with all of our team.

We work in neighbourhood models which align with other services to best meet the needs of the community. Our staff teams are welcoming, friendly, supportive, and innovative, encompassing and valuing all our team members. Our visible Leadership team is positive in driving service improvement in a compassionate and collaborative way, working with all teams to ensure we deliver high-quality care in a supportive team environment.

You will provide compassionate, community-based support to children, young people, and families in Tameside, Greater Manchester, helping them stay healthy and avoid unnecessary hospital visits. You will be responsible for a caseload of children and deliver the Healthy Child Programme within a team. To ensure children receive the best start in life and school readiness is promoted.

You will lead and manage the delivery of the Healthy Child Programme, work with complex cases including safeguarding. Identifying at the earliest opportunity children with Special Educational Needs and Disability—signposting and supporting children, young people, and families—whilst being leaders in public health. You will work in collaboration with our neighbourhoods, communities, voluntary, and statutory agencies. We deliver the Greater Manchester Early Years Delivery Model and are committed to improving our school readiness rates.
