Tower Hamlets GP Care Group (the Care Group) was incorporated as a community interest company limited by shares in September 2014. Every general practice in Tower Hamlets owns one share of £1. The Care Group was formed to manage borough level contracts on behalf of the practices, deliver primary care led services and support general practices, and to be the provider voice of primary care in the wider health and social care system.
The CareGroup is also a member of Tower Hamlets Together, a place-based integratedpartnership, which includes Bart's Health, East London NHS Foundation Trust, theLondon Borough of Tower Hamlets and the Council for Voluntary Services.
The key priorities for the Care Group are:
Deliver the Healthy Child Programme, conducting mandated contacts and providing tailored support to families.
Work autonomously and as part of an integrated skill mix team, ensuring seamless service delivery.
Contribute to safeguarding children and young people, adhering to local and national guidelines.
Collaborate with multi agency professionals to address complex needs and improve outcomes.
Facilitate and support service development and change initiatives.
Effectively manage a caseload, including universal and targeted populations, ensuring equitable service provision.
Provide clinical supervision and mentorship to colleagues and students, fostering a supportive learning environment.
Maintain accurate and up-to-date records, adhering to data protection and confidentiality policies.
Participate in health promotion activities and community engagement initiatives.
