We are seeking a highly motivated and enthusiastic individual to effectively deliver and promote our Health Referral Scheme. This is an exciting new role with the opportunity to redevelop and manage the scheme across our Freedom Leisure sites in the Ashford Contract.
To monitor and evaluate the scheme effectively, you will work closely with the Healthy Communities Manager & our Fitness Manager to ensure correct delivery, administration, and promotion of the scheme.
As a Health Referral Coordinator, you will oversee referral scheme activities and lead the development and implementation of programs, including cancer rehabilitation, dementia care, and menopause movement sessions.
You will build and manage key relationships and cross-sector partnerships across Freedom Leisure, public health, and community, voluntary, and third-sector organizations.
