An established industry player in health services is looking for a dedicated Health Practitioner Assistant to join their multidisciplinary team. This is an exciting opportunity for an experienced Health Practitioner Assistant/Health Care Assistant to join the Integrated Community Reablement Assessment Service (ICRAS).
Essential Criteria: NVQ Level 3 - Health & Social Care.
We have multiple posts available across South Sefton and Liverpool, providing care and support for patients who need assistance to stay at home instead of hospital admission, or who require support after hospital discharge to ensure their assessments for care are conducted in their own homes.

The successful candidate will have opportunities to learn, develop, and practice various skills, becoming a valued member of our multidisciplinary team.
Communicate effectively with the team and other agencies, undertake further training, and deliver patient care aligned with standards and policies. Document care accurately in patient records.
