Job Description

The Health Improvement Worker will work as part of the Local Mental Health Team and wider PCN to deliver physical healthcare interventions to patients with Severe Mental Illness (SMI).

The post holder will have a specific remit to support achievement of the physical health standards for people with SMI by engaging service users in routine physical health checks and support the person to work towards personal physical health goals in order to improve their physical and mental health.

Responsibilities include:

  • Supporting and engaging with people with Serious Mental Illness to have their cardio-metabolic risk factors measured.
  • Carrying out observations and taking blood samples in order for risk factors to be identified.
  • Supporting patients in understanding their risk factors, how to manage their risk factors and agreeing a plan with the patient for ongoing monitoring and management of the risks under the supervision of a registered clinician.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
