This position is designed to contributing to avoiding hospital admission, facilitating early discharge from hospital and maintaining patients in their own homes. This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services, as well as the community and voluntary sector.

The Health Connector will be a member of the Health Connectors service which works across the locality (Fareham, Gosport, SE Hants and Portsmouth). Health Connectors will work with individuals and groups utilising untapped community resources, support the facilitation of self- help groups and other innovative approaches to enable people to better manage their own health and wellbeing.
