Diabetes Prevention Practitioner

Join us on our journey to empower over a million people to change their behaviours and thrive!

At Thrive Tribe, we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier. It’s an exciting time to join us as we expand our delivery of the Heathier You NHS Diabetes Prevention Programme throughout South East London. We’re looking for passionate health coaches (or Diabetes Prevention Practitioners) to join our team!

The purpose of a health coach is to improve lives by empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight, and lower their risk of developing diabetes.

Like all roles at Thrive Tribe, you can expect your work to be fun, varied, and challenging. We adapt roles as needed to allow our team to play to their strengths.

A typical day as a Health Coach might include:

  1. Delivering evidence-based face-to-face group sessions focusing on diet, nutrition, physical activity, and behaviour change across South East London.
  2. Facilitating group interventions in community venues to support clients in achieving their goals in a positive environment.
  3. Making service user calls, triaging, and onboarding new patients into the programme.
  4. Promoting the Healthier You NHS Diabetes Prevention Programme internally and externally, and improving referral pathways.
  5. Engaging in ongoing training, submitting your annual CPD portfolio, and evidencing professional development.
  6. Working towards criteria and targets within the NDPP quality assurance framework, including all Clinical Governance procedures.
  7. Contributing to new projects and service developments across Thrive Tribe services.
  8. Adhering to internal policies, including Information Governance, Safeguarding, and Complaints procedures.
  9. Driving our organisational culture and embedding our values.

The role may require some evening or weekend work.

Experience in delivering group-based interventions and understanding how personal circumstances impact health behaviours is important. Bonus points for qualifications in health promotion, nutrition, exercise, or health coaching. Fluency in Portuguese, Urdu, Tamil, Spanish, Gujarati, Farsi, Korean, or British Sign Language is also desirable.
