We have an exciting opportunity within our Urgent Response Team. The Urgent Response Team (URT) is a multidisciplinary team of advanced clinical practitioners, nurses, therapists, and social workers who deliver urgent response services to Salford residents.
This role involves working as part of a team utilizing technology and new ways of working to provide a safe alternative to hospital admission and support early discharge from hospital. The role is expanding to include the Hospital and Care at Home (H&C@H) virtual wards model of care.
This is an opportunity to shape services that improve healthcare experiences for Salford's population and align with NHS national priorities.
It is essential that the post holder can meet the travel requirements of the role. Holding a full UK driving licence and access to a vehicle is desirable.
Please note: Changes to UK immigration policy as of 9th April 2025 have increased the minimum salary threshold for sponsorship from £23,200 to £25,000. This role does not meet the criteria for sponsorship due to salary constraints.
The role involves establishing and maintaining communication with patients about routine activities, assessing health and wellbeing needs, and supporting patients in achieving personal hygiene, mobility, nutrition, and independence.
