Qualifications

Job Description:

An exciting opportunity is available to join our Overnight Response Service, providing an unplanned specialist advice line and responsive visiting service to palliative care patients and carers overnight.

This role involves working closely as part of a team of 2 with a Clinical Nurse Specialist.

You will answer telephone calls, undertake home visits with the specialist nurse, provide supportive patient care, and input details into SystmOne, along with other duties to support the service's efficiency.

Access to a pool car will be available, and driving is a key component of the role.

The pilot service is supported by access to specialist palliative medical advice as needed and operates between 22:00 and 08:00, 7 days a week.

You may be required to work some day shifts to maintain skills and meet training requirements.

Applicants should be highly motivated, experienced in providing palliative and end-of-life care within the community, and possess empathetic communication and interpersonal skills.
