Overview
Band 3 Health Care Assistant, working with IV Therapy Team. Successful candidate would be based at Breightmet Health Centre (subject to change). Clinics are based at Breightmet Health Centre; domiciliary visits are across the borough of Bolton. Successful candidate must have access to their own transport.
Responsibilities
- Venepuncture – in clinic and during domiciliary visits.
- Clinic duties – assisting with clinic, general duties within clinic setting, running bloods and weights clinic.
- Admin – assisting with admin where required.
- Stock – ordering stock and maintaining stock levels.
- Collecting medication from hospital – taking to patients' homes or returning to Breightmet Health Centre.
- Taking bloods to hospital.
- Work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual care plans relevant to patient need within their practice limitations.
- Assist registered staff in clinic – bring patients to and from waiting area, perform and accurately record patient observations, provide patient with refreshments, and clean equipment between patients.
- Communication – communicate effectively with all patients and their relatives; coordinate with the team and wider MDT as needed (e.g., haematology, gastroenterology, AAT).
- Stock management – maintaining adequate stock levels, ordering stock via NHS supplies and Integra ordering systems.
- Admin support – assist during annual leave of team admin (LE2, CIVICA, preparation of notes, preparation for clinic).
- Support the team flow – provide support to the team leader, team coordinator, and other members as appropriate; support discharge flow from hospital to the IV Team and print notes in absence of admin; prepare stock equipment for new patient visits.
- Collection of medication from pharmacy – bring back to base or deliver to patients' homes.
- Documentation – effectively documenting patient notes and updating clinic records.
- To support the daily running of the team while working within scope of practice at all times.
- To provide support to the Admission Avoidance team where necessary through collaborative working.
