Job description
Overview

Health Care Assistant role in the community within the NHS for Bedfordshire. You will deliver patient-centered support and compassionate care to people over the age of 18 living in Bedfordshire.

You will be part of a multidisciplinary team (MDT) with clinical and managerial support, aligned with social care professionals in reflectio n of the NHS 10-year plan.

Responsibilities
  • Deliver quality care to a designated caseload in the place the person calls home, following a care plan devised by the community nursing staff.
  • Provide concise information in a daily handover to ensure changing needs are met and risks are escalated.
  • Collaborate within a wider MDT to support integrated care delivery.

  • Flexible to cover service needs across the wider integrated team
  • Full induction programme provided
Details
  • Location: Mid Bedfordshire (Ampthill, Shefford/Biggleswade) and surrounding areas; community nursing teams in Biggleswade, Sandy, Potton and surrounding villages, and Ampthill (Flitwick, Barton, Woburn and surrounding Mid West Beds villages)
  • Working pattern: 08:00–21:00 hours with rotational shifts including weekend, evening and Bank Holidays; part-time posts considered
  • Access to a car to carry out service duties is essential
