Job summary

A leading health care provider in the UK is looking for a Band 3 Health Care Assistant to provide compassionate care across Hull & East Riding. The position involves travelling to deliver care in patients' homes and coordinating with local healthcare colleagues. Overview

We are recruiting for a Band 3 Health Care Assistant within the West Hull Nursing and Conditions Team. This is a full-time position of 37.5 hours.

Responsibilities
  • Provide care in the community across many sites within the Hull & East Riding area, travelling between sites and delivering care in patients\' homes and other settings.
  • Use your own vehicle to travel safely and effectively; have a UK driving licence with business use insurance and a vehicle suitable for business purposes.
  • Work across Hull & East Riding as required; a base will be identified but you may be asked to work anywhere within the area.
  • Support long term conditions management, complex wound care and phlebotomy as part of a holistic, quality community nursing approach.
  • Work in a multidisciplinary team and coordinate care with other local providers and healthcare colleagues in Hull and the East Riding.
Qualifications/Requirements
  • Excellent communication skills and an adaptable approach to respond to dynamic changes in community nursing.
  • IT literacy.
  • Willingness and ability to travel across Hull & East Riding; operate using your own vehicle; maintain a UK driving licence with business use insurance; vehicle suitable for business purposes.
  • Full participation as part of a team delivering care in patients\' homes and other community settings.
