St Christophers Hospice was founded in 1967 by Dame Cicely Saunders, and formed part of the modern hospice movement. You will be able to support patients and families with both their emotional and practical needs, within the comfort of their own homes.

We are looking for an emotionally and physically resilient individual, to join our small Choose Home Team - Supporting those whom wish to remain at home (across five London Boroughs), at end of life.

This involves providing community-based personal care as part of a short-term emergency bridging service, until FastTrack (or alternative) arrangements are sought. This can also include visiting people at points of care crisis, with no equipment or microenvironment yet in place - working creatively, to remain solution-focussed. Choose Home also provides a variety of ad-hoc interventions, including: Occasional last offices, skill-sharing for families/other care staff and transitional support following recent discharges from the hospice inpatient unit and local hospitals.

To succeed in the role:

The successful candidate(s) will be compassionate, flexible, reliable and motivated in providing a high-quality care service toward our local communities. As well as having: A good level of education- Including NVQ L2/3 in Health and Social care, or equivalent- And recent experience of working a HCA role (EG. Providing personal care) within a Health and Social Care environment, or similar.

You will be confident working both independently and as a team, including the ability to establish good working relationships across the broader organisation.
