We are a largeteaching and training practice and pride ourselves in excellent patient care. Weare continually adapting to changes in Primary Care through innovation. We havean exciting opportunity for a dynamic and enthusiastic Health Care Assistant.

The Health Care Assistant will be a member of thepractice nursing and administrative team, providing high quality care to thepractice population under guidance from a practice nurse mentor. The post primarily supports doctors andnurses in basic clinical tasks, involving blood tests, blood pressure checks,wound dressings, chronic disease management, infection control and stockcontrol. To provide clerical support when needed.

Main duties of the job

CLINICAL DUTIES

To carry out a primary health care assessment onnewly registered patients that will include urine testing, blood pressure check, height, weight, body mass index calculation andappropriately record medical history in patients medical records.

To observe and report all matters pertaining topatients care changes in condition/circumstances to a relevant member of thenursing or medical team as a matter of priority and ensure that a documentrecord is kept.

To ensure that the clinical rooms are at all timeskept in a clean and orderly manner and that relevant equipment is ready for useby nursing and medical staff during clinic sessions.

To assist in clinic sessions as directed by thenursing team or GP ensuring that all equipment is clean and sterilisedfollowing clinic sessions.

To assist with minor operations including dressings.

To assist with coil fitting.

To undertake home visits to housebound patientswithin surgery catchment.

To ensure that the drug refrigerator is at correcttemperature on a daily basis and that drugs are within their expiry dates andused in correct sequence.

To ensure adequate levels of stock are availablein the surgery.

To act as a chaperone during patient consultations

To administer vaccines under a PSD followingappropriate training.

You will act at all times with regard to the confidentiality and sensitivity of issues to do with patients,their care and treatment and staff and doctors. All health related contacts with patients must berecorded on the patient record using the appropriate read- code.

To administer call and recall systems in relation to the practice QOF (Quality Outcome Framework) targets as and when required.

Undertake necessary audits as and when requested in line with best practice.

Work with your nurse lead, nursing team and operational manager to ensure that the nurse team aspires to best practice.

You will be supervised, and given guidance by members of the relevant nursing, medical and clerical staff throughout your term of employment under the direction of your practice nurse mentor.
