Job Title:Primary Care Health Care Assistant (HCA)

Responsible to:Assistant Clinical Director, Haringey GP Federation

Hours of work:Part time 3 days per week Fixed Term

Location: Head Office is currently based at 48 Station Road, Wood Green, London, N22 7TY Exact location will be within the Enhanced Access Hubs in Haringey, with potential for some Outreach and out-of-hours work.

Main duties of the job

The aim of this post is to increase access to primary care services and improve the outcomes of both physical and mental health for these patients. The post holder will do this by carrying out health checks that can identify each persons risk of developing heart disease, stroke, type 2 diabetes, and kidney issues.

Increasing the proactive identification, monitoring and management of physical illness in this patient cohort.

Improving engagement of Haringey patients with primary and community care, including referral or signposting to appropriate services.

Supporting practices to maintain accurate records on the specific patient cohort.

Identifying and coordinating patient care with the secondary organisations supporting the service.

Job responsibilities

Provide physical health checks, management of health conditions advice and support as well as holistic support (where appropriate).

Assist in the triage of referrals using local protocols to ensure timely clinical review and increase patient access to specialist advice and treatment.

Assist and support using clinical assessment skills and clinical judgement to take a detailed history, physically assess, record previous diagnoses, and tests/investigations, record current medication being taken and support in the development of a care plan.

Support colleagues in undertaking clinical assessments in partnership with patients and other professionals as required.

To provide, a phlebotomy blood sampling point of care testing POCT service for patients to maximise the likelihood of completing a health check within a single appointment.

Be responsible for processing and management of laboratory samples requested by clinicians with specific responsibility to ensure that the samples are correctly labelled sent to the laboratory promptly and that data is entered onto the patient's notes.

Be responsible for the correct maintenance and cleaning of equipment.

Be responsible for restocking of all clinical areas and maintaining a stock rotation system to the required levels as agreed with the clinical team.

To immediately report any safeguarding or compliance concerns.

Participation in all activities required by CQC registration.

To ensure ones own skills and knowledge are kept up-to-date, and evidence based.
