Job description

Purpose of The Job

As a member of the Millie college staff team to provide a high standard of health and social care to a student or group of students to enable them to achieve their maximum potential. To cooperate closely with all members of the multi-disciplinary team and external agencies. This role will be either working 1:1 or 2:1 with a student as well as working in a group over communal activities.

Main Duties

Clinical

  1. Implement existing health care program to enable the students to live as independently as their disability allows, retaining their individuality, dignity and respect.
  2. To implement care / health action plans in partnership with the student, family and carers, whilst working in close co-operation with other health professionals.
  3. Liaise daily with the Care Lead regarding the welfare and appropriate services for the student(s).
  4. Promote self-care and independence, where appropriate and prepare the student for independent living whenever possible.
  5. Maintain safe medication storage, recording and administration in accordance with the NMC (Nursing and Midwifery Council) medicines management guidance documents.
  6. Monitor and evaluate standards of care and report to Care Lead as necessary.
  7. Monitor and evaluate daily personal documentation as required i.e. Fluid balance charts, bowel charts, weight charts.
  8. Work with all members of the multi-disciplinary team to ensure that each allocated student has a healthy, well balanced diet, management of incontinence and other independence training as agreed with the Care Lead.
  9. Communicate with the Senior Team if a student becomes ill and requires emergency services or transfer to hospital.
  10. Report and record all incidents using the incident reporting system. If a student has an accident or an incident occurs. Via Leadership Team and using safeguarding process support the referral to GP or Hospital as required. Provide daily communication to students’ families as directed.
  11. Maintain medical equipment to conform with Health and Safety legislation, and Infection Control. Maintain stock control.
  12. Contribute to the implementation of the different stages of all acute and progressive care plans.
  13. Report and document relevant health care activities in designated daily/periodical logs.
  14. Obtain and record vital signs as necessary/indicated in appropriate logs.
  15. To use a range of communication strategies to support communicating with students with a range of complex needs.
  16. To undertake other duties when required such as supporting Students in class setting, taking lead from Tutor/Co-Tutor.
  17. To understand and support all aspects of the Enterprise, Curriculum and Future pathways.
  18. To assist students with eating/drinking during mealtimes and care during the lunchtime period including those students with Dysphagia (training will be supplied).
  19. To support all students with all Personal care as required. This will involve Moving and Handling (training will be supplied).
  20. To support all students with independence and Life Skills. This will include following plans set by the SALT, Physio and Occupational therapist, under teaching and supervision.
