Job description

We are excited to offer an opportunity for a compassionate Health Care Assistant to join our expanding Community Investigation Hub, based at Burnham-on-Sea War Memorial Hospital. This service is part of North Sedgemoor Primary Care Network (PCN), which brings together five GP practices to deliver joined-up care to our local population.

Main duties of the job

You will support a range of investigations and monitoring services within a structured clinical team. This includes taking bloods, performing ECGs, spirometry, routine observations and vaccinations.

Most clinics run from our dedicated hub at Burnham-on-Sea War Memorial Hospital, with some travel to member GP surgeries and occasional home visits. You will work to defined protocols, supported by senior clinicians, and play an essential part in delivering high-quality care to patients across the network.

Job responsibilities

This role is based within the Community Investigation Hub (CIH), a service delivered by North Sedgemoor Primary Care Network (PCN). The post holder will support the safe and effective delivery of a range of diagnostic and preventative care services to our local population.

Working under defined protocols and within your level of competence, you will assist the clinical team in providing high-quality investigations and monitoring, with a primary focus on blood tests but including a broader range of diagnostic tasks.

Candidates must be able to do at least 16 hours per week, with flexibility up to full-time.

Key Responsibilities

Carry out phlebotomy procedures in line with local policy and professional training

Support delivery of ECGs, spirometry and other observations, ensuring accurate documentation

Administer vaccinations as required within competence and training

Maintain clean and safe clinical environments, adhering to infection control policies

Record activity appropriately in clinical systems and ensure data is entered accurately and promptly

Communicate effectively with patients, carers and colleagues

Work independently under guidance, planning and prioritising your daily workload

Participate in ongoing training and maintain professional competencies relevant to CIH services

Attend relevant PCN meetings and contribute to the development of the service

Be willing to travel across CIH sites and member GP surgeries as required

Additional Responsibilities

Support the CIH Lead Nurse and Project Manager in monitoring service delivery and quality

Participate in audits and quality improvement activity

Maintain patient confidentiality at all times, in accordance with PCN policies

Report safeguarding, health and safety or infection control concerns appropriately

Be adaptable to changes in the service model and take part in service development activities

Qualifications
  • NVQ Level 2 (or equivalent) in Health and Social Care
Experience
  • Previous experience working as a Health Care Assistant
  • Proven phlebotomy skills and recent practice
  • Ability to follow protocols and work to clinical standards
  • Clear communication skills and ability to work well within a team
  • Good organisational skills and ability to prioritise tasks
  • IT literate, with a basic understanding of clinical systems
  • Understanding of confidentiality, safeguarding and infection control
  • Commitment to providing high-quality, person-centred care
  • Willingness to work across multiple sites
  • Experience working in primary care or community health settings
  • Training or experience with ECGs, spirometry or vaccinations
  • Familiarity with EMIS Web
  • GCSE (or equivalent) in English and Maths
