Location: Reigate

Salary: Level 4 banding £40,334.00 Pro Rota 22.5 hours per week.

Service Type: Care and Support leadership Team Central Office

We are seeking a dedicated Health and Behaviour Facilitator to enhance our support services and address health inequalities. The role is within our higher level 4 management banding, requiring robust experience in health and behavioural support. You will empower individuals to lead fulfilling lives by ensuring access to healthcare, supporting screenings, and providing information for health-related decision-making. This role offers a chance to create meaningful change and challenge the status quo in primary care.
