Diamond Health Group are seeking a motivated Health and Wellbeing Coach to join our skilled multi-disciplinary team, to deliver a high standard of patient care. The role will involve identifying the needs of patients; supporting them to take pro-active steps to set goals and achieve their personalised health and care plan objectives.
This position is for 30 hours per week and the working pattern can be discussed further at interview.
