Job description

EAST:MK Primary Care Network has an exciting opportunity for a health and wellbeing coach to join our practice team, on a full-time basis, for 37.5 hours per week. You will work over 5 days, between Monday and Saturday, on a rotation basis across our 4 surgeries: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice, and The Grove Surgery, which make up the PCN.

You will collaborate with the multidisciplinary team (MDT) within the PCN to ensure all patients receive the best care and service. The role focuses on supporting patients to feel in control of their lives and to make behavioral changes towards their personal goals.

Your duties include coaching and motivating patients through multiple sessions, identifying their needs, setting goals, and supporting them to implement personalized health and care plans. This empowers patients to manage their health and well-being, live independently, and improve health outcomes.

Main duties of the job
  1. Care planning and delivery of self-management support based on activation levels.
  2. Care coordination across the care continuum, including supporting carers.
  3. Supporting effective team-working in primary care, including attending team meetings and working with complex-care teams.
  4. Delivering group and one-to-one sessions on healthy eating, physical activity, and wellbeing, aligned with care plans.
  5. Engaging patients using health-coaching techniques to promote active participation in their health.
  6. Supporting practices with NHS Health Checks, Smoking Cessation, and outreach programs.
  7. Raising awareness of support groups and resources to help patients achieve their health goals.
  8. Collaborating with clinical and non-clinical colleagues and the wider PCN to optimize patient outcomes.

Person Specification
Experience
  • Experienced health and wellbeing professional confident in motivating and advising diverse patients.
  • Reflective, self-aware, and attentive to patient needs.
  • Knowledgeable about managing long-term conditions and health behavior change.
  • Experienced in developing personalized care plans and writing concise assessments.
  • Completed or willing to undertake PCI-accredited health coaching training and Tier 1 practice/expertise.
  • Excellent communication skills and strong local community knowledge.
  • Experience in health promotion, nutrition, health advising, or training.
  • Familiar with clinical systems like SystmOne and social prescribing.
Qualifications
  • Coaching, counseling, or relevant health/wellbeing qualification or experience.
  • Qualification in health and wellbeing, mental health, or physical activity.
Additional

This role is subject to a DBS check under the Rehabilitation of Offenders Act 1974.

Salary: £29,000 per year minimum, dependent on experience.
