Overview
We have an exciting full time opportunity for you to become our Health & Wellbeing Coach. The role is hybrid and the office is based in Hatfield, so you need to drive and have your own transport. The role involves supporting families, so experience of working with children/young people and delivering on our Youth and 1:1 services is required. The role will also involve working at least 2-3 evenings a week, with latest delivery until 19:30.

The Health and Wellbeing Coach provides personalised support to individuals to empower them to take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of:
- Healthy Eating and Nutrition
- Physical Activity
- Weight Management
Responsibilities
- Facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life.
- Deliver face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one-to-one and group-based support in line with need, location and offer.
- Provide health coaching for eligible participants that supports people to gain and use knowledge, skills, and confidence to reach self-identified health and wellbeing goals, including but not limited to: triage and assessment; agenda setting; lifestyle behaviour change support (e.g., stop smoking, move more, healthy weight/diet, reduce alcohol); problem solving; motivational interviewing; goal setting; signposting.
- Deliver services in multi-channel ways and personalise support to individuals and groups, including face-to-face, telephonic, and digital coaching; use multi-mix communication methods (in-app messaging, text, email); share helpful resources aligned to goal and journey stage; advocate behaviour tracking tools; assess caseloads daily to ensure appropriate intensity and frequency of support; apply a strength-based approach and utilise local community assets.
- Support outreach and community engagement work (e.g., NHS Health Checks, community events, external meetings, roadshows, pop-ups); identify and escalate where coaching is not appropriate and support onward referrals to wider services.
- Work in line with business and contract performance requirements, including SOPs, policies, quality management (data protection, information security, Health and Safety, safeguarding), audits and performance observations, and ongoing professional development.
- Engage in supervision and training with a commitment to personal development.
Requirements
- Experience of working with families, children, and young people; delivery of Youth and 1:1 services.
- Full-time availability with flexibility to work 2-3 evenings per week (latest 19:30).
- Driving licence and own transport for a hybrid office role based in Hatfield.
- Ability to deliver across multiple channels (face-to-face, telephone, digital) and to work with diverse communities.
- Strong communication, motivational interviewing, goal setting, problem-solving, and signposting skills.
