A local government authority in Croydon is seeking a dedicated Health and Wellbeing Assessor. This role involves working closely with a multidisciplinary team to support individuals in regaining independence. Ideal candidates should be committed to person-centred practices and confident in working autonomously. We’re looking for a dedicated and knowledgeable Health and Wellbeing Assessor to join our LIFE (Living Independently For Everyone) service. This role is key to helping individuals regain independence and live fulfilling lives at home.
You’ll work closely with a multidisciplinary team including Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses, supporting them by carrying out delegated assessments and interventions. You’ll be based in the community, hospital, or people’s homes, working independently across a variety of settings.
