We’re bringing together nurses, therapists, health and therapy assistants to work alongside acute multi-disciplinary teams and transfer of care hubs, promoting efficient hospital-to-home transitions for our patients.

Our multi-disciplinary Home First teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.

Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions at home to support recovery and prevent re-admission.

Home First also provides acute reablement for up to 10 days, helping individuals achieve functional goals and regain independence in daily activities.

As a Health and Therapy Assistant, you will provide personal, health, and therapy care for people across Sussex with diverse needs. Your day will be varied and rewarding, supporting individuals to achieve greater independence.

You will assist multidisciplinary colleagues in assessments and care provision, including clinical procedures like taking observations, venepuncture, administering medications, wound care, and rehabilitation activities. You will observe changes in patients’ conditions and communicate updates to your team.

No previous experience is required; we will train you on the job, supporting your development towards the Care Certificate through our comprehensive competency framework.

You need to be able to drive with a full UK license and have access to a vehicle, due to the geographic coverage of the team.
