The Home First service brings together nurses, paramedics, therapists, health and therapy assistants to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs. The aim is to promote an efficient, effective discharge from hospital to the patient’s own home.
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions in their own homes to support recovery and monitor health post-discharge to prevent re-admission. Where needed, Home First provides acute reablement for up to 10 days, supporting individuals to achieve functional goals and work towards independence with daily activities.
The principle of the service is to ensure that recovery involves working towards the person being as independent as possible through enablement.

You will provide personal, health, and therapy care for people across Sussex with diverse needs. Your day will be varied and engaging, supporting individuals to achieve greater independence.
You will assist multidisciplinary colleagues with ongoing assessments and care provision, including procedures such as taking observations, venepuncture, medication administration, wound care, rehabilitation, and equipment provision. Observing changes in patients’ conditions and maintaining accurate records are essential.
No prior experience is necessary; we will provide on-the-job training, including basic nursing and therapy skills, and support you to achieve the Care Certificate. Our comprehensive competency framework will support your continuous development.
With experience and additional training, you may advance your career towards roles such as Nursing Associate or other degree-level healthcare professions like Nurse, Podiatrist, Midwife, or Therapist.
