Job description

Responsibilities:

  1. Conduct assessments and develop personalised care plans to meet each patient's unique needs.
  2. Serve as a support system for patients and families, providing ongoing care coordination and emotional support.
  3. Complete holistic needs assessments that address physical, emotional, and social aspects of care.
  4. Educate and mentor staff and learners, promoting best practices in head and neck cancer care.
  5. Conduct comprehensive Holistic Needs Assessments at key points along the cancer pathway.
  6. Assess, plan, implement, and evaluate nursing interventions to meet patients' needs.
  7. Collaborate with the multidisciplinary team (MDT) and attend meetings to ensure patients' wishes are prioritised.
  8. Under the guidance of the Head and Neck Clinical Nurse Specialist, provide specialised nursing care and support to patients with Head, Neck, and cancer, along with their families/carers.
  9. Support patients across various care settings, including wards and outpatient clinics.
  10. Act as an advocate for patients, families, and carers when needed.
  11. Assist patients in achieving self-care and educate caregivers to support them at home or in the hospital.
  12. Empower patients to manage their health and well-being, ensuring they can express their preferences, worries, and concerns.
