Adult & Aging Services - Dickinson, Watford City, and Surrounding Regions, ND - HCBS Case Manager

Sign on bonus of up to two (2) months’ salary with a commitment of 2 years of work.

The ND Department of Health & Human Services (HHS) Adult & Aging Services Division is seeking a Home and Community Based Services Case Manager to assist individuals in remaining in their own homes with supportive services. This position is remote and will serve individuals in Dickinson, Watford City, and surrounding areas.

As our HCBS Case Manager, you will provide case management to elderly and disabled individuals under the Home and Community Based Services Program. Responsibilities include performing assessments, developing and implementing individualized, person-centered care plans, monitoring progress, and maintaining records. You will also coordinate referrals with community partners.

To succeed in this role, you must be proficient with computers and possess strong time management and organizational skills. The position requires visiting clients at their homes or facilities and traveling to conduct on-site assessments.

Qualifications: You must hold a valid licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners or as a Baccalaureate of Nursing with RN licensure (BSN/RN) by the North Dakota Board of Nursing. Additionally, you need at least one year of professional human services experience in roles such as social worker, counselor, case manager, nurse, or similar, or a Master’s Degree in social work with licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.

If no applicants meet these qualifications, consideration will be given to those who have passed the ND Social Work licensure exam and are awaiting their license number, or those with LBSW and less than one year of experience.

Location: This role is remote with travel to Dickinson, Watford City, and surrounding areas.
