Job description

The Community Mental Health Team is seeking a skilled, motivated, organized, and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing on individuals with severe and enduring mental health problems. The successful candidate will be responsible for care coordination and case management of a defined caseload of clients with complex needs, as outlined in Parts of The Mental Health (Wales) Measure.

Proficiency in Welsh is desirable; applications from both English and Welsh speakers are welcome.

Main duties of the job
  • Coordinate care for a designated caseload of clients with severe and enduring mental health issues.
  • Ensure adherence to Care and Treatment Plans and compliance with the Mental Health (Wales) Measure.
  • Act as a liaison for GPs and interface services, providing advice, support, and signposting.
  • Develop standard and complex outcome-focused care and treatment plans.
  • Complete comprehensive risk assessments for actual and potential risks.
  • Support clinical duties, including assisting with depot/clozapine clinics and supervising long-acting injections.
  • Conduct physical health assessments and monitor side effects for patients on Clozapine.
  • Assess individuals in crisis, evaluate health and social care needs, and determine appropriate actions.
  • Formulate care plans and deliver evidence-based crisis interventions collaboratively with service users, carers, and partner agencies.

Proficiency in Welsh is desirable; applications from both Welsh and English speakers are encouraged.
