Job Summary

RUH Hospital at Home provides intensive hospital-level care for acute conditions that would normally require an acute hospital bed, delivered in a patient's home for a short episode by a multidisciplinary healthcare team. The service aims to offer a better person-centered care experience, reduce healthcare-acquired infections, and prevent institutionalization. The team provides short-term intervention, medical recovery, and rehabilitation at home, maintaining hospital standards to support patients' independence and dignity.

Main Duties of the Job
  1. Assess, diagnose, plan, implement, and evaluate individual treatment interventions using expert clinical judgment and close liaison with the multidisciplinary team.
  2. Utilize expertise and decision-making skills to inform clinical reasoning when dealing with complex and varied presentations, synthesizing information from multiple sources for evidence-based judgments.
  3. Exercise professional judgment to manage risks appropriately, especially in complex and unpredictable situations, supporting teams to ensure safety for individuals, families, and carers.
  4. Model trust values by demonstrating a person-centered approach to service delivery and development.
  5. Continuously evaluate personal practice and participate in multidisciplinary service and team evaluations, demonstrating the impact of advanced clinical practice on service quality and effectiveness.
