Job overview

We are looking to recruit two dynamic and innovative pharmacists to join the Medicines Management Team. One opportunity is based at GP practices across Hammersmith & Fulham North PCN, and the other is a rotational post across Hammersmith & Fulham North PCN and our rehabilitation units.

The postholders will gain experience in conducting structured medication reviews, carrying out long-term condition consultations, providing a high-quality clinical pharmacy service, and working within a multidisciplinary setting to provide specialist expertise.

The postholders may also be required to travel to other sites and services within the organisation, requiring flexibility as the organisation continues to grow. A car would be useful for traveling but is not essential.

Main duties of the job

The post holders must have post-registration experience in either hospital or community pharmacy and/or GP practices. Previous experience in community health services or primary care is desirable. A postgraduate qualification to at least diploma level or working towards or completed the CPPE Primary Care Pharmacy Education Pathway, as well as an independent prescribing qualification or currently working towards completion of this, is desirable.

A sound knowledge of prescribing and medicines management issues faced in both primary and secondary care is essential, along with excellent decision-making and problem-solving skills. Additionally, compassion, flexibility, excellent communication, and time management skills are essential, as well as the ability to build relationships with various health and social care professionals.

  • Undergraduate Degree in Pharmacy
  • Professional registration with the General Pharmaceutical Council (GPhC)
  • Takes personal responsibility for Continuing Professional Development as evidenced by portfolio
  • Uses reflective practice for personal and professional development
  • Completion of CPPE Primary Care Pharmacy Education Pathway (or working towards)
  • Independent prescribing qualification (or working towards)

Experience
  • Post-qualification experience of clinical pharmacy, medicines information, and prescribing advice
  • Experience of working in secondary care and in a multidisciplinary environment
  • Primary care experience including Community Services
  • Experience in supervising a team/service and line management
  • Experience of training others
  • Experience of audit techniques
