East Lothian Hospital to Home Team
We have exciting opportunities for Healthcare Support Workers within our Hospital to Home service, creating a dedicated team to support people to stay longer at home in partnership with local health and social care services.
Your role involves assessing, planning, providing, managing, monitoring, and evaluating nursing care in a community setting, including assistance with daily activities, medication, meal prep, and end-of-life care.
What are we looking for?
Qualifications: Basic literacy skills, good education, sufficient English language skills for safe practice.
Personal qualities: Good health, reliability, observational skills, initiative, teamwork, enthusiasm for elderly care, supportive references, no restrictions affecting duties.
Note: A full driving license is essential; a fleet of cars is available for staff use.
