A leading healthcare provider is seeking an experienced Practice Nurse to join their dynamic team. The role involves delivering high-quality care, managing chronic diseases, and providing health screenings and education to patients. Applicants should possess NMC registration and good experience in general practice.
We are looking for an experienced Practice Nurse to join our practice team. The successful candidate is someone who is motivated and committed to providing high quality care, working collaboratively with the general practice team to meet the needs of our patients.
Previous experience of working in general practice isessential.
Provide nursing treatments and procedures to patients in participation with the GPs or independently to agree protocols
Providing assessment, screening and treatment services and health education advice
Participate in Chronic Disease management to practice protocols, referring to or liaising with the patients usual GP as needed
Provide general and specific health screenings and assessments to the practice patients within agreed protocols liaising with the GPs as necessary concerning risk factors or other items identified
Triage patients and treat those with minor illness as needed
Arrange for and perform appropriate tests as indicated such as blood, swabs etc.
Make referrals as necessary to other health and social care professionals
Hounslow Family Practice is a partnership of two GPs, providing care for 6,100 patients in Hounslow. Our exceptional clinical team includes 2 GP Partners, 2 Salaried GPs, a Clinical Pharmacist, 1 Practice Nurse, 1 Physician Associate, 1 clinical pharmacist and 2 Health Care Assistants. This clinical team is supported by an administrative staff, which includes a Practice Manager, an Administration team, and a Reception team, all dedicated to serving our patient population.
