Job description

Telephone assessment prioritisation of all patients who contact the Out of Hours Service with urgent medical needs and face-to-face assessment prioritisation of patients streamed to the Urgent Treatment Centre from A&E.

GPOOHs works when GP surgeries are closed, covering evenings, weekends, overnights, and Bank Holidays. This is a multidisciplinary team supporting patients of West Cheshire GP surgeries and visitors to the West Cheshire area.

We offer a relocation package of up to £8,000**. The package is available for those needing to relocate to take up employment with Cheshire and Wirral Partnership. Terms and conditions apply.

Key Responsibilities:
  1. Using clinical decision software to assess all presentations to the Out of Hours Service, prioritising calls while maintaining good public relations and promoting effective timely access to care.
  2. Assessing patients streamed to the Urgent Treatment Centre from A&E and referring them to the appropriate healthcare setting (e.g., Primary Care Unit, GP Out of Hours, Extended Hours, community services, and home care advice).
  3. Ensuring contemporaneous record keeping within the embedded clinical decision software.
  4. Communicating effectively using observational and active listening skills to record comprehensive patient histories.
  5. Making autonomous clinical decisions while managing unpredictable workloads.
  6. Providing evidence-based healthcare advice, maximising opportunities for health education and promotion.
  7. Organising the prioritisation and flow of patient referrals to the multidisciplinary team, highlighting and recording significant events.
  8. Highlighting safeguarding risks identified during triage and initiating adult and child protection policies in line with Trust guidance.
  9. Participating in the management of the 24-hour final days pathway and in cases of sudden and expected deaths.
