GPOOHs works when GP surgeries are closed covering evenings, weekends, overnights and Bank Holidays. This is a multidisciplinary team working to support patients of West Cheshire GP surgeries and visitors to the West Cheshire area.

Main duties of the job
- Using clinical decision software to assess all presentations to the Out of Hours Service, prioritising all calls whilst maintaining good public relations and promoting effective timely access to care.
- Using clinical decision software to assess patients who are streamed to the Urgent Treatment Centre from A&E and referring them to the most appropriate healthcare setting (e.g. Primary Care Unit, GP Out of Hours, Extended Hours,community services and home care advice).
- Ensuring contemporaneous record keeping within the embedded clinical decision software.
- To communicate effectively and skilfully with the use of observational and active listening skills and dialogue to ensure that comprehensive patient histories are recorded.
- To be accountable for autonomous clinical decision making whilst managing unpredictable workload.
- To provide evidence based healthcare advice, maximising opportunities for health education/health promotion.
- To organise the prioritisation and flow of patient referrals to the multidisciplinary team of call operators, drivers, nurse clinicians and doctors, highlighting and recording significant events.
- Responsible for highlighting safeguarding risks, identified in the triage process and initiating adult and child protection policies in line with Trust guidance.
- To participate in the management of the 24 hour final days pathway and in sudden and expected deaths.
